Survivorship

In 2014, the American College of Surgeons Commission on Cancer mandated in Standard 3.3 that survivorship care plans (SCPs) be provided for all patients at the completion of active cancer treatment. Minimum requirements for what the SCP should include were provided; however, no standardized model was proposed.
The Northwell Health Cancer Institute consists of 12 disease site Centers of Excellence, one being the Breast Cancer Center of Excellence.
Cancer and its treatment can have long-lasting or late-onset effects. A robust survivorship program, founded on distress screening and survivorship care plans as recommended by the Institute of Medicine and Commission on Cancer (CoC), can offer support, guide treatment, and provide education in the years after completing cancer treatment.
Cancer survivorship is rapidly evolving, with new cancer survivorship care guidelines issued very recently. Stay up-to-date on new information to ensure that practice keeps pace with new guidelines for care.
